To show our appreciation for your efforts, we will provide you with a long sleeved commemorative t-shirt/goodie bag and post event refreshments! All individual participants raising $250 or more will also receive a commemorative Chicago Polar Plunge towel! Individuals raising $500 or more will receive a commemorative Chicago Polar Plunge jacket which will be mailed after the event.

OPTION A
Step 1 - Click here to create an individual or team fundraising page through which will allow your friends and family to make online contributions. Quick and easy! You can send emails to all your friends and family members. No follow up administrative work on your end - donors will receive an acknowledgment notice/tax deductible receipt and you will receive an email when donations are received.
Step 2 – Pick up your commemorative t-shirt and goodie bag at North Avenue Beach beach house located right at North Avenue and Lake Shore Drive
You can also check in and pick up your t-shirt/goodie bag on event morning at North Avenue Beach from 8:30am-9:45am
OPTION B
Step 1 - Download Registration Form, print out registration form and pledge sheet.
Step 2 – Complete your registration form and mail (by February 22) or fax to to us today at:

Step 3 - Keep pledge sheet and collect pledges all the way up to the event!
Step 4 - Turn in pledges, pick up your t-shirt and goodie bag at at North Avenue Beach beach house located right at North Avenue and Lake Shore Drive
Pledges may also be turned in on event morning at North Avenue Beach from 8:45am-9:45am.

Register in person, pick up your t-shirt/goodie bag and turn in your pledges at North Avenue Beach beach house located right at North Avenue and Lake Shore Drive
OPTION D
Join us on event morning between 8:30am-9:45am at North Avenue Beach, register and turn in your pledges just before taking the Plunge!
